Academic Suspension appeal letter

Dean and Faculty,
I am writing to appeal for re-entry into Fitchburg University in the Psychology Bachelor program. I had been attending school consistently until health issues forced me to be absent from several classes. Prior to missing classes, I had been experiencing pain in my throat, difficulty swallowing and talking, a fever, and fatigue. The symptoms and pain I was experiencing were unfortunately limiting my ability to focus and concentrate on my school work. I attempted to resolve the issues with a visit to the school nurse, on September 27, 2017, who recommended that I seek additional medical care. It finally reached the point where the pain was so severe that I sought emergency medical attention at the UMass Memorial Hospital in Worcester on October 3. 2017.My diagnosis at the Emergency Room was Peritonsillar Abscess which resulted in them having to make an incision in my mouth to drain the abscess. Based on the level of pain I was experiencing, the potential for infection, and the fact that I wasn’t getting any relief from the procedure performed at the Emergency Room, I made the best decision for my health at the time; I returned home for a short time to follow-up with my primary care physician and attempt to rest and recuperate.
Three days after the initial procedure, I again found myself in the Emergency Room, this time at Baystate Medical Center where I was seen by the nurse and the attending Ear, Nose, and Throat doctor, Dr. Donald Einck. Once again, the procedure to drain the abscess was performed as the doctor indicated that it hadn’t been completely removed by the previous procedure. This caused an increase in my pain symptoms. The next couple of weeks were spent taking multiple antibiotics and pain medication and recovering from the diagnosis as well as both procedures.Once I returned to school, I was still plagued by some residual pain and a lot of fatigue which continued to interfere with my studies. I followed up with the nurse when I returned to school. As I was having difficulty taking the medication with the pain in my throat. The nurse re-prescribed one of the antibiotics as I left it at home during my return to school.
I am now back to my normal self and I am committed to my studies and performing well in all of my classes. My plan is to focus on my time management and ensure I am studying daily for my classes in order to achieve academic excellence. My goal has never changed from the first time I decided to attend Fitchburg University. This was not a decision made in haste, this is my life dream that I am determined to accomplish and I want to accomplish it with Fitchburg.
I hope that I am granted the opportunity to prove that I am worthy of continued attendance at Fitchburg. I appreciate your time and consideration.

Also I have documents from both hospitals & from the health services at my school for a total of 38 different documents

This was also my first semester at school
