I consistently volunteered for an organization every week during my junior year, and I plan to continue volunteering there in the spring of my senior year. I’m not currently volunteering there because of time constraints and issues with transportation. Is it okay for me to check on Common App the “12th grade” box, even though I plan to continue it in the spring (after college apps are due)?

I wouldn’t. It’s not really accurate to claim to be doing something you’re not actually doing. Will anyone fact check you? Probably not. But you should be telling the truth.

How is this different from indicating that you plan to participate in a spring sport in 12th grade, for example?

Good point. I guess a spring sport only takes place in the spring so it’s clear that it is not a current activity. A volunteer opportunity that was previously done every week but isn’t currently being done is different. But again, that’s my view of it. Will it be fact checked and scrutinized? Probably not.
