<p>I had the very same question when I applied last month. I emailed the Office of Undergraduate Admissions and they replied that the activity chart is a portion included in the Northwestern Application (or the Common App, depending on what you choose to apply with). It’s the area that you fill in with extracurriculars, community service, summer camps, etc, and where you include the number of hours per week devoted to each activity as well as any leadership positions you might have held.</p>
<p>Basically, when you fill out and submit your application, you’ll have submitted the activity chart, since it’s a part of the application.</p>
