If you are planning to be a music major you will have to audition. There is a forum especially for music majors on this site so I suggest you visit that to find out more.
If you are not planning to be a music major, then playing an instrument is an extracurricular activity that should be listed in that part of the app. Certificates don’t matter. You can just say that you play and put in any additional information like are you part of a band or ensemble and how many hours you spend on it per week.
