<p>One of the few things that you need to do as an incoming freshman is to fill out something called an Alcohol Assessment Form or Survey. The link to it was in the incoming freshman application site myapplication.ucsd.edu. Now the site is closed. Does anything know how to get to the Alcohol Assessment Form?</p>
