Applying for Optometry School with academic misconduct NOT on transcript

Okay, so I am going to straight out say i messed up. One of the biggest mistakes of my life. I was at a university and during summer vacation i applied as a guest student to a different university and took a class there. Now i messed up. I was stupid and took everything as a joke and was caught cheating, so i withdrew from the course, but was charged with academic misconduct. I retook the class at my normal university and aced it and i never transferred the class.

I posted this before and received advice to look at my official transcript. After requesting a copy i found it did not mention anything and after talking to several counselors i found out that it is in a different record, not part of my transcript. The counselor there told me that since it’s not part of my transcript i should only mention it if optometry schools ask for my academic history. I looked online and it seems that they only ask for transcripts, but i am unsure if this is the right thing to do.

If the optometry schools ask for “all transcripts” then you are fine, supplying the transcript fully complies with the school’s request.

If they specifically ask “have you ever been subject to any disciplinary actions at any schools you have attended,” then you’d have to report the incident.

Grad schools typically ask, so you had better come up with a good addendum.
