<p>On the second sheet (front side) of the health care form, there are 5 fields of information including height, weight, P, R, and blood pressure. What does the P and R stand for? Thanks in advance.</p>
<p>just got my physical yesterday… and my doctor filled in that part (so i’m hoping that ur doc will fill that in and know what it is). otherwise, i cannot find my health form @ the moment (i think my mom sent it in).</p>
<p>i remember looking @ that and seeing ##/### <– some number over another… i THINK (not 100% positive) that P = pulse; R i cannot remember but i’ll ask my mom and post later.</p>
<p>p is for pulse, r is for respiratory effort. obviously these are things you actually have to go to the physical to put down, but I’m pretty sure the form has to be signed by a doctor anyway, so that shouldn’t be a problem.</p>

<p>i think the “p/r” is for blood pressure or something.</p>
<p>Since blood pressure is a separate thing, I’d say that p/r is pulse and respirations.</p>
