Those are all I can recall off the top of my head.
What are my chances of getting into Duke with these credentials?

I’d appreciate any input; thanks in advance.

As someone who transferred into Duke from Rice two years ago, I think you have a good chance. However, it is extremely competitive to get in as a transfer student; Duke accepts between 3-7% each year. You will need to first ask yourself : Why do you want to transfer? What does Duke offer that Carolina doesn’t? Keep in mind that Carolina has excellent academics and some of its departments are better than at Duke; you might even find the math and computer science departments better at UNC.

In your application, you will need to show that Duke offers you something more than Carolina does. Prestige can’t be reason. Prestige is a terrible reason to transfer, especially since you are doing well at the Honors Program in Carolina.

Good luck!
