What are my chances of acceptance for top tier schools, i.e. Ivy League and top one hundred schools, and what colleges should be in my target range?

Expected Major: English
GPA 3.4(cumulative), incurred three concussions freshman year, had bad effect on GPA (2.4 Freshman Year, 2.8 Sophomore Year, 3.6 Junior year.)
ACT: 25 Composite with no studying or prep. 30 English, 27 Reading, 25 Science, 17 Math. Retaking 3 more times, planning on 30+
Taking 2 AP classes senior year and two honors
Extracurricular: Three years of football, three years of track, one year of wrestling, two years of rugby. Avid writer. Attending FCA, also joining a distinguished writing program that requires application and worth college credit
200+ hours of community service. Never suspended or incurred detentions.
Note: First two years I attended a top 50 private school. Also cannot play football because I broke my neck last year, so no fourth year of football.
You’re not really a candidate for highly selective schools.
