Changing 2nd choice major

After applying or being admitted, some students decide that they would prefer a major other than the one to which they applied or have been admitted.</p>

<p>To request a major change, you must submit a request in writing (not e-mail). The request must include the applicant’s full name and signature, date of birth, and application ID. (There is no guarantee that your request will be granted.)</p>

<p>Do i just write it on notebook paper?</p>

<p>Also, is it as simple as </p>

<p>"Dear Admissions, This is Bob Dole (DOB) (App ID) and I would like to change my 2nd choice major on my application to XXXXXX?</p>
