Cover page for additional rec letters?

<p>My friend was deferred and is sending additional rec letters from the principal and an alumni. Since these letters won’t have a form accompanying them, should he include a cover page with name, DOB, SSN, etc. to make sure they can file it quickly and correctly? If so, what should it include?</p>

<p>I think it should include all the information that the usual rec forms ask for.</p>
