<p>If a title adds something to your essay, use one. If it’s just a lame title like “My Journey” that does nothing to enhance it, you aren’t required to have a title.</p>
<p>Formatting is all up to you. I’d recommend being professional and clean; pink size 20 Comic Sans in yellow highlight isn’t going to do you any favors in how you present yourself. Just be smart about it; formatting won’t cost you admission so need to stress. You’ll never go wrong with Times New Roman size 12. Either way, admissions people do have lives, and will not obsess over your font, spacing, etc. as long as it isn’t offensively sloppy/hard-to-read.</p>
<p>Name? I’d include it just because. Maybe even as a header with the page number after it (i.e. in MLA).</p>
<p>You don’t usually need a title. However, you should mark each page with your name, SSN, and the type of essay it is (personal statement, addendum, etc)</p>
