Do supplemental recommendations require the teacher evaluation?

<p>I’m asking my art teacher to write my 3rd, supplemental rec for HYPS. Does she also need to fill out the common app evaluation form?</p>

<p>She doesn’t need to for HYPS; a letter is fine. The only school I know of that wants supplemental recs on the common app form is the University of Chicago.</p>

<p>Does anyone know for Brown?
And would the employer just send it in as a separate mailed letter?</p>
