<p>I would direct this question to the admissions counselor at the particular institution you are applying to. Some may have a specific form online. With the point of making it as simple as possible for your teacher, since she probably gets asked a lot. But every institution may be slightly different - which is why your teacher is asking you to find out. </p>
<p>Also a good idea to ask your teacher for a copy (signed) for your files. And permission to copy and send it to another school if and when you have more applications. </p>
