When a person applies and would like to let the university know he took AP classes, would he miss out on credits for AP exam, or both?

Each University will have a list of class credit it will give you for certain scores on the AP TEST.

So if you took AP Bio and got a 5 and the Uni says they will give you credit for Bio 101 with a score of at least 4 then you get credit for the class.

Most people take an AP Class in order to prepare for the AP test but I am not sure if it is required.

I feel a headache in my head now, because the Johns Hopkins prerequisites website says that they don’t accept online AP classes. So what does that has to do with the exam? Do they even specify which classes were taken?

I wouldn’t worry about AP classes, you can self study for AP tests.
Here is what Johns Hopkins will accept:

http://apply.jhu.edu/apply/examcredit/

They only give credit for AP tests not the class.

And you usually need a 4 or 5 on the test for credit.

You should also check your individual degree requirements, some limit how much AP credit you can apply. For instance OU awards credit for many AP exams but if you are going for a degree in Meteorology, no more than 25% of the credits used for your degree can come from AP or CLEP credits (around 32 credits)
