<p>Hey, question about the Elks Most Valuable Student Scholarship Verification of Student Enrollment form.
Should I put my name or my college name for the “make checks payable to”? And who (which title/position) would be my school official? If this person is at my future college, then I will not be able to get his/her signature. And also, where can I find the school IDs? Is it just the one Collegeboard uses?</p>
<p>These answers are assuming that this year’s form is the same as last year’s:</p>

<p>You should put the name of your college as the check can only be made payable to a university. If you look under the “Special Instructions to College/University/School” section on the lower half, it says that the check will not be made payable to the student under any circumstances. Plus this form is supposed to be filled out by a school official (see the orange instruction sheet).</p>

<p>You should take this form to the Registrar’s office at your university. It will usually be someone working there who will fill out your form. Typically they have a stamp with the signature and title of the university registrar. I am assuming you have not moved to your school yet. To fill out the form, you have two options: one, you can send/fax in your form (make sure you keep a copy for yourself) and then call them to ensure receipt and to walk through the process with them. Two, you can wait until you have fully matriculated into the university at which point you can go to the Registrar’s office for the signature and enrollment confirmation. Since checks will not be mailed until after September 1 (assuming this is still the date like last year’s), you have plenty of time. Plus if you send it in now, you will most likely not be matriculated into the system yet (meaning no official enrollment confirmation).</p>

<p>I am assuming you are referring to the section on the form where it says, “Please provide your school Student ID.” When you’re matriculated into your university, you’ll receive a student number (if you haven’t already). That is the number they want so they can make sure the check is deposited to your account. It is not the same number Collegeboard provides (which is actually the school code). The student ID should be on your student ID card if you have one already. If not, check with the school for your information.</p>
