Not always. This is very college-specific.
But for the OP, I think it is simply that the reader misconstrued something from the application. Although to be fair, the applicant really should have listed both Albanian and English as first languages unless s/he was raised in an Albanian bubble with no TV/videos until attending school. If so, s/he would be well-served to make the change for any applications not yet submitted. An applicant born and raised in the US has no need to take TOEFL for any college that I am aware of.
Also, do you think I should update my common app and change the language?
If yes, should I make them both my first language or just English?
