<p>Are you going to fax the consent form? I asked the girl (I was calling to sign up for the program) if we have to fax it like the pamphlet said to, but she said no; just mail it.</p>

<p>I was directed by the Registration Office to fax the form, and also to mail it along with the $45 check. Hope that helps!</p>
