<p>Yeah, just make a document that lists your activities, your participation level, and a brief description of the club/activity/your duties (if you had any).</p>
<p>So for example it would be a list of something like:</p>
<p>JV Volleyball: 10th-12th grade, XYZ position
Spanish Tutoring Club: 10th-12th grade, tutor younger students once a week, vice president 11th grade, help organize events
Piano Lessons: 9th - 12th grade, take private lessons and participate in competitions once per term</p>
<p>Or what have you. And the descriptions would be best as full sentences, but you get the idea. Once you write your document out just email it to OAFA.</p>
