I have a single biological parent and a donor who is not known by neither my parent or I. How would my parent go about filling out all forms? It’s a sperm donor thing and I can’t fill out the waiver as it requires the noncustodial parent to even be there. Do I just contact the school with the information?
Yes. Just send a copy of the birth certificate, and an explanation that you have no known bio father.
