MIT Student Information Review Form</p>
<p>Loan entrance counseling and the Perkins and Stafford promissory note are forms you have to read and sign before they’ll disburse your loans to you.</p>
<p>The student information review form is for you to be matched with a particular donor’s scholarship – the donor might have asked that the scholarship go to someone from a particular high school, or who did a certain extracurricular activity.</p>
<p>I don’t know the exact purpose of the credit balance authorization form.</p>
<p>but do we have to fill it out and then send it in? </p>
<p>Is it required for our FA package? I haven’t got mine yet from MIT. or do we fill out this form after we decide to matriculate at MIT?</p>
