<p>Because the report says “Counselor or Teacher”, I would be a little hesitant. I would probably suggest submitting two reports: one from a Counselor or Teacher and the other from your registrar. This question is worth a call to the admission office before you do it.</p>

<p>If you didn’t get the barcode labels in the mail, look for your ID on any mailings you received and write it where the barcode would go. If you haven’t gotten any mailings, use your SSN.</p>
<p>Admissions says you can also put your name and birthdate in that spot if you don’t want to use your SSN.</p>
