<p>I’m going out of the country for the next 2 months, and i’m concerned about the health form that needed to be turned it. according to the “booklet” that came with the acceptance letter, it stated that the form must be turned in before registering. I’m wondering registering for what? Classes? and When do we register for classes??? O_O Also, on the website, there is this response form that they said you need to turn in. Is that the same one as the one in the form that came with the acceptance letter??? the Meningitis thingy…</p>

<p>I just looked at the booklet that came with the acceptance letter (online as well) and I don’t see a specific date for the form to be submitted to health services…We just assumed that it had to be submitted by orientation. If you see something different, please post…There is no meningitis “thingy” on the form that came with the acceptance letter, but there is a box to check off that you already had a vaccination within the last 10 years…(which I think is identical to the one posted on the health website you noted)…</p>

<p>Also, depending on your school (ie Newhouse, A&S, Whitman) you should be receiving a registration packet any day now…</p>
