HELP How should I report these ACT Scores?

Applying to College Common and Coalition Application

SO here are my 2 scores:

  1. Dec ACT: E/M/R/S=35/34/31/36+36 WRITING=34 Composite
  2. Feb ACT: E/M/R/S=36/35/34/35+30 WRITING=35 Composite

Please note that I am applying SCEA to Yale, so they require all scores anyway. Should I put both scores on the Common App testing section? And then when I send my ACT Score Report, should I send both test date scores or just one??
