<p>Please help me with this… The Middlebury Admission office sent me a letter saying that I need to submit signed statements of gross income, benefits, and etc from my parents’ employers…
What kind of form is this? 
Has any of you guys written it before? If so, would you please pm me the form…? I am in a desperate need…since the financial deadline has passed 1 month ago and my family cannot afford the university education without financial aid.
