How does medicare part d work?

My FIL was recently diagnosed with chronic leukemia and bladder cancer. He is on Medicare. He has both part d and part b coverage.

The bladder cancer treatment plan is not worked out yet, since one of the tumors is hard and calcified.

However, the leukemia treatment plan is ready and involves taking a drug (don’t know the name). From what I understood the drug is not on formulary of their part D insurance plan. However they can work through either process of exception or with manufacturer of the drug to drop the price of the drug to $2.5k per month.

Assuming that the process of exception works and drug is approved by insurance company, I have a hard time believing that my in-laws will be expected to pay $2.5k per month for a drug (in addition to office visits, etc.). Their combined SS will be less than that.

I think my MIL does not understand something. Can someone in the know clarify how is this supposed to work?
