How many times to sign the "I agree not to cheat... blah blah blah"?

<p>Does anyone know how many times do we need to sign the “I agree not to cheat… blah blah blah”. (I can’t remember the rest)</p>

<p>I remember seeing it 2 times per test, but I can’t remember if I signed it for 1 time or 2 times.</p>

<p>How many times do we need to sign it? Is it 2 times per test?
Is it 1 time for the english, math, reading and science, and 1 time for the writing?</p>

<p>Twice. One on the answer sheet and another one on the test booklet </p>

Did you sign them at different times per test? Is it one at the beginning of the test and the other is at the end of the test?
Cause I can’t remember if the teacher had asked us to sign the second one.</p>
