Could someone link to an imgur or something of a mock/fake filled out non-filer tax form? That would help the most instead of miscommunication through text. I’m confused as to how to even fill it out properly. Do I enter my name into the 1a section or the 3 section where it asks for current name and then the address? The 1a section seems to require that you be ordering a tax transcript, so I assume that’s not the place for me to put my name. I have no clue, it’s so unclear. The directions are awful.

I would put your name in box 1a, even though you are not requesting a tax transcript. You need to put your SSN in box 1b, because the IRS will check that it has no return on file with that SSN, and the SSN needs to be accompanied by your name. Also put your name and current address on line 3. Checking only item 7 for the product requested will be enough to show that all you want is a Verification of Nonfiling
@BelknapPoint Ok, thanks. It seems the only thing I need to correct is adding my name to the address in label 3. What if I lived somewhere else in 2015 though? Shouldn’t I put my previous address in label 4? Also, do I fax the form to my previous state or my current state? Sorry for all the questions.
Have you previously filed a federal tax return, using an address that’s different than you current address? If not, than you leave line 4 blank.
The form doesn’t get sent to a state; it gets sent to the IRS, a federal agency. Mail or fax the form to the IRS address or fax number that corresponds to the state where you presently reside, as found in the instructions for the form.

No, I haven’t previously filed one. Sorry for all the pestering. The reason I asked about which state was because it was dependent on whether or not I would have to fill in label 4. Of course, you’ve cleared that up for me now so thank you.
