College board will mail you grades to said college with everything they need to identify you.

When you choose a school to send your SAT scores, the scores are sent along with the identifying information you gave to the college board when you signed up for the test. That includes plenty of information to particularly identify you. It includes name, address and high school and possibly other items like race. When you send your scores to a school, if the scores are the first item the school receives from you the school will open an application file for you and add other pieces of your application as they arrive.
