I submitted a withdrawal form for my foreign relations class and now i have a problem

I go to SFSU, and i submitted a petition to withdraw from a course which is international relations class. and i submitted the form 5 weeks before the semester ended, 5 weeks before dec 20. I was told by the department assistant that i would get the W online on my grades in a about a few weeks, However, i never recieve any confirmation or grades for the past month ever since i submitted a form. and I find out today i got a letter grade of “D” for the course, i had no idea if the Withdraw form was denied/approved/incomplete. Was i suppose to to pick up the form at some point to confirm my withdraw, Did they assume, that i dint go to the department and ask how the form was being processed, and proceeded to deny my Withdraw from the IR course? I never got an email, or anything. The withdraw policy for the university does not include when you are suppose to pick up or check on the Form. Would i have to get another kind of withdraw, called retroactive withdraw from the course, next semester, can this D be changed to a W somehow? i dont know which forum to put it on? For some reason the Professor thought i was still registered in the class, maybe thats why i got a d? I even talked to him during the semester, and he approved my withdraw from the class?

The only people who can answer your questions are those who work in the SFSU registrar’s office (or whatever other office was supposed to process your withdrawal). Stop in to speak to them the first chance you get.

i will at the first thing of the semester, the thing is that i was supposed to get a w, but somehow ended up with a d. I talked to a friend who also goes to sfsu, and she said, they shouldve sent me some sort of confirmation. I have some theories though, its possibly they either lost the form, they forgot about it, or they never even checked the department mailbox.=

It looks like the drop deadline was 9/9/2013.
<a href=“Welcome to the Registrar's Office | Registrar's Office”>Welcome to the Registrar's Office | Registrar's Office;
However sometimes with your profs approval you can drop course after the deadline, but I am guessing you didn’t say anything to, or get a signature from your prof?
Perhaps you can change course to incomplete depending on what you would have to make up.

No no, the dropdeadline for 9/9/2013 is for dropping without a W on your record, any withdraws after that date until the 13th week is with a W on the record. the problem is i never heard back from the department of Ir, after i submitted the petition to withdraw. The assistant told me i would see a grade a week later, he told me to put it in a specific mailbox, most likely the department chair, however it could been another mailbox, which they ignored the request. This was right before thanksgiving. There is a chance i can get another kind of withdraw, retroactive withdraw, which means i can withdraw from a class from the previous semester, if for some reason i was unable to withdraw from that class that semester, with good serious reason, but this is just speculation.
