However, she has told me that she did not get a W-2 form at all and that the income was merely reported on the 1040 tax return. What should I put on IDOC? You need to report the amount of income she earned, which she clearly knows because she included it in the 1040. What kind of work did she do, and did she report it as regular income or was it reported on a Schedule C (business income)?

If your mom didn’t get a W-2 or 1099, she should have either a check stub or deposit records, something like that…you can upload screenshots or scans of those. Plenty of companies don’t provide 1099s when they are supposed to. You could also contact a staffer at one of your CSS college’s financial aid offices and ask them what the recommend.
And sometimes a 1099 is not required if the amount paid to the person is less than a certain amount…by any employer.

Not belknappoint, but if a company pays someone >$600 in any given year, they are supposed to give that person a 1099. If a person doesn’t get a 1099, they are still required to report their earned income on their tax return…as OP’s mom seems to have done.
