<p>^When I went, I just took a printed out copy of all of my immunizations and they accepted that.</p>
<p>I did the waiver for that particular vaccine, so I’m not sure if it’s different, but for all of my other vaccines they simply accepted the record that I got from the Health Department.</p>
<p>So, I’d imagine whatever your doctor’s office gives you will work.</p>
