Immunization form question

<p>So I’m looking over the Immunization Record Form, and I see the following in Section A (Required Immunizations), Question 2:</p>

Menningococcal vaccine:Required for all incoming students age 21 and under.</p>

<p>Proof of receipt of a conjugate meningococcal vaccine or a signed waiver declining the vaccination is required for all incoming students age 21 and under.

What constitutes “proof of receipt”? </p>

<p>D’s doctor filled in the vaccine dates, is that enough proof?</p>

<p>^When I went, I just took a printed out copy of all of my immunizations and they accepted that.</p>

<p>I did the waiver for that particular vaccine, so I’m not sure if it’s different, but for all of my other vaccines they simply accepted the record that I got from the Health Department.</p>

<p>So, I’d imagine whatever your doctor’s office gives you will work.</p>
