What happens if a teacher or guidance counsellor doesn’t fill out a form in time? What are you supposed to do? One of my guidance counsellors has 4 days to fill it out and he procrastinates a lot!

Just remind him and maybe encourage him to go faster

Do your best to get him to submit it on time, but do it with as much tact as possible. If he does not get it in on time, email your interviewer (assuming you have already interviewed), and tell him or her that GC has the form and you are working on getting him to complete it.

Depends on your school. If your child is at a large public school, chances are they don’t really care and you should let admissions know. If it’s a smaller school, do try and talk to them.

Get all of your pieces in on time. Schools generally understand that recs are beyond your complete control. You can always contact your AO to let them know the situation if you think he won’t make the deadline.
