Letter of Recommendation Form online kind of confusing (or not)

<p>I might be dumb… but the information they’re asking in the first part (name, address, etc…) would be the teacher information right? </p>

<p>In addition I know it is optional, but wth is “relationship to you”, is this for students with a teacher that is a parent or something? </p>

<p>Well I’ll take a gamble here to try to help.</p>

<p>Name, Address etc: Ms. J. Smith, So and So High School @ this address</p>

<p>Relationship to You: History Teacher</p>

<p>BUT : somewhere on this online form of course the teacher should re-state YOUR name as in:
" I am honored to recommend Lee Cheng So and So High School Class of 2012 for UMD-CP…"</p>
