<p>I don’t think this is anything to worry about. Write a short note–more like a cover sheet–with your name, address, phone number, name of high school, the date you applied, etc.
Make a notation such as: Additional Info to Include with Previously Filed Application: High School Profile.</p>
<p>Put this in an envelope. When you get back to school after the holiday break, bring it to the guidance office and ask them to attach a copy of the high school profile to the cover letter and mail it out for you.</p>
