Do we have to fill out the info for my daughter’s father when she has had no contact with him for years and I have both physical and legal custody (as in he legally has no say in her life)? We never include him on anything else and we don’t know the answers to all the questions.
I do not want to contact him. We’ve been divorced for ten years and I’m still terrified of him.
