Not sure what to focus on

I’m a sophomore at a decent state university looking to transfer to a top-tier school such as Berkeley or an Ivy. I am pre-med with ambitions to do research as an M.D./Ph.D. GPA is ~3.8 (SAT 2100+). I slacked off in high school but now I picked up the pace.

My question is this: what of all things should I prioritize in order to get into such a selective school? I am working on research (my primary focus right now), classes, extracurriculars, sports, trying to make connections, and trying to establish a start-up business. Thing is, all of these things take up a lot of time. I am really committed to transferring, but I need some help deciding on what I should focus on at this point in the game.

Thanks in advance!

One thing a lot of transfers forget about is recommendations. You need two plus recs from college profs and these are weighted higher in the transfer process than the regular process. This is especially true for non-state schools where they’re looking to “shape” a class.
