All NROTC scholarships are highly competitive. You would first need an undergrad degree, then you’d have to compete for the med school scholarships. Some other posters will be able to give further info on PT requirements and ROTC scholarships.
I do not know much about Navy opportunities for physical therapists, but physiology (and “related sciences”) is listed below as one of the approved Tier 2 majors for the NROTC scholarship:
