<p>What was the name of the worksheet that UCSD made you download prior to confirming & accepting the federal pell/perkins grant award?</p>

<p>I saw the instructions and a hyperlink next to it which was in a .PDF file (form) but I went too fast and check accept award. I tried to go back to the previous page again but the link to the .pdf file to get the form was gone.</p>

<p>Anyone has the link to it or know the name of the form from the .pdf file that you have to complete and send it back to UCSD ?</p>

<p>I dont think there was any form or nething for the Pell Grant,for the perkins loan u hv 2 fill out a Promissory Note,u can access it by going to ur official award like u did the before,this time,it will be written ready for disbursement on xyz date,n below that itll be written “documents neede” or “steps to b completed”,the perkins promissory note link is rite there.</p>
