You know they used to, don’t you? I had to submit photos to most colleges when I applied.

Harvard dropped photo requests in 1958, because the state of Massachusetts ruled that to be illegal discrimination, according to this archived The Crimson page: University Not to Seek Photos on Applications | News | The Harvard Crimson

But Georgetown was requesting photos until recently at least.

AFAIK adding a photo to one’s Georgetown app is optional, and has been for awhile.

Separately, plenty of high school transcripts still have a photo of the student. I have no idea why parents would allow that.
