PLAN score converted to ACT score?

i recieved a 26-30 on my ACT range when i took the PLAN, but i took it in 9th grade, i also took it with a severe concussion with left me technically mentally disabled (seriously) i was wondering if anybody had any experiences with their plan score converting to the actual ACT and how much of a change could i hope to see? i know the question is general but thanks in advance for any possible help!

In 9th grade I took the PLAN test and got a 29 out of 32… And as a sophomore, I took the ACT and got a 32 (I bombed the reading section haha)

It said on my score report 29 on PLAN means 30-33 in ACT
