I have increased my GPA by 4 points in just one year. I finished junior year with a 97, and so far I have a 101.1 average senior year. If colleges see my improvement since I have been diagnosed with ADHD and they see my high test scores, do I have a chance at making it in?

Don’t send your SAT!
I think you have pretty good chances; the process is holistic so they understand learning difficulties. make sure your essays communicate.
