Pre-Matriculation Health Record Form

<p>I either never received this form in the mail or accidentally tossed it out, but my friend said that this form is due Monday, May 21. I tried calling the clinic office but couldn’t get through several times, so I just emailed them instead requesting a new one. Do you guys think that the 21st is the final date for this to be turned in, or can I perhaps turn it in during summer orientation or something?</p>

<p>That phone number sucks. The phone never even RINGS.
So, I emailed them too.
Turning it in during orientation should be fine…that’s what I’m doing.
They bar you from registration until you turn it in…so as long as you turn it in before registration, you should be good.</p>
