"Prison bars" over a poster's icon?

What does it mean when a poster has bars over his or her icon? Today I noticed them over a well-respected poster’s icon.

I’ve also wondered what they mean.

Usually a “major warning” isn’t given for a first offense. It most often happens when a user has repeatedly broken the rules.
