Ok so basically I got a write up in 5th grade, but in my school for whatever the 5th graders are in the middle school building and are technically middle schoolers. So I was wondering if I should mark that I did get a disciplinary sanction on the gateway prep application or not.
My reaction reading this is that if you respond yes AND that it was in grade 5, nobody will care and you’ll come across as honest. You’ll also eliminate the possibility of getting caught up in the scenario in which it’s on your record but you didn’t disclose it and then having to explain why.
Curious what others think.

My first thought is to look for the definition of a “disciplinary sanction” for the purpose of the application that you’re filling out. And then find out if the 5th grade write up that you mentioned fits the definition.

It wouldn’t hurt to contact admissions at the schools that you are applying to and ask. They may say not to count that because it was 5th grade.
Even if you are told to check “yes”, whether or not a school would view this as a red flag would be dependent on what the reason was for the disciplinary sanction.
