My school did not hand out any “senior packets” for counselor and teacher recommendations. Supposedly some schools have a questionnaire for seniors that is handed in along with a resume to help teachers understand the student better when writing a recommendation? Also, do you have to get an individual recommendation for each college that you apply to, or does it send to all the colleges that you apply to?

Then create your own. It’s basically just a brag sheet with your stats, activities, honors, etc, to give to your recommenders to potentially add some texture that they did not already know about you.
If the colleges are on the common app, the teacher writes one recommendation for all colleges, although you may choose not to have his/her recommendation sent to each college. For schools not on the common app, the teacher will often cut and paste what s/he wrote for the common app.
When you talk to teachers about recommendations, ask if there is anything you can give them that would make it easier for them to write it. In our HS some teachers give students a form to fill out, one of my D’s teachers asked for her resume, one of my S’s teachers asked him to provide a list of EC activities he was involved with, and others didn’t want anything.

One at most. UC’s, as an example, want no counselor recommendation. Each school’s website should give detailed info about what it wants included in the application packet.
