questions about school forms

<p>For the instructor evaluation, should I put my home address as the return address or let my instructor put their own address?</p>

<p>For the school official’s report, is it okay to just give my adviser one form and ask her to make multiple copies? Also, should I be the one mailing out this form or should I provide my adviser with addressed envelopes?</p>

<p>I gave each instructor and my advisor a copy of the approrpiate form, with a stamped envelope addressed to each school I am applying to with their return address (eg office address at present school). I did not think it approprate to have my return address as I had signed waiver that I will not request to see what they wrote.</p>

<p>I usually recommend that you put your own return address on everything because if there is a problem (eg. D1s school sent the wrong forms to schools), then it comes back to you and you can take care of it, rather than going to the college or instructor, who do not have a vested interest in submitting a compete application.</p>

<p>I agree about signing the waiver and trying not to have to look at the form/LOR, but in this case I think it’s more important that you assure that everything is delivered.</p>

<p>I go to a very.very.small school and see these guys every day, so it works for me. They will let me know if it comes back.</p>

<p>Since I go to a very large school, I guess it’s a better idea to put my home address then? But if I do that, would the adcoms think that I read the LORs after signing the waiver?</p>

<p>No, just have your instructors sign over the seal of the envelope.</p>
