Registration form question - URGENT help plz

Hey guys I just nooticed that in the form in the SAT test where you have to put your name, signature, home adress, date of birth, sex and all of that, the number 6. square says: Registration number (Copy from Admission Ticket), and at the lower part it shows a circle and it says “I turned in my registration form” today, should I mark the circle? what is the registration form? is it the same thing that im putting my info on?

thanks

also, is ZIP code necessary? , my admission ticket doesnt say anything about my zip code

and what mean “mid.init” in Name/Surname box?

The ticket you’re bringing to the test (that you printed from the computer) has the registration number. You only fill in the circle about turning it in today if you’re a standby tester (meaning you didn’t register online, you didn’t pay anything, you just showed up at the testing center on the morning of the test).

I always fill in the zip code, because they ask for it. I’m guessing withholding it would delay your score, although I have no confirmation on that.

Middle Initial (aka “mid. init” or “M.I.”) is the first letter of your middle name.

For item 6, fill in the circle only if you are a standby and don’t have a Registration Number. Be sure to fill in the bubbles.
