Required Forms

<p>I’ve been looking into forms that are required by the following university:</p>

<p>Johns Hopkins University (JHU)
University of Michigan
University of Wisconsin-Madison (UW-Madison)
Boston University (BU)
Purdue University
University of Illinois Urbana-Champaign (UIUC)
University of North Carolina, Chapel Hill (UNC Chapel Hill)
University of Pittsburgh
Ohio State University (OSU)
University of Massachusetts, Amherst (UMass Amherst)</p>

<p>I know it seems like a varied range of Universities, but I am a Massachusetts resident. However, if there are also other forms for international students, please please please also let me know. Some of the university’s applications are by Common App. However, there are cases in which universities require a separate form. Please tell me what forms, how to submit (mail or online), and the deadline. </p>

<p>For an example, JHU requires international students to submit their Financial Verification Form by January 1 through mail.</p>

<p>The answer is in front of you. You go to each colleges’ website and SEE FOR YOURSELF if they require additional forms beyond what’s in the Common App. Do you expect someone else out here to do it for you?</p>

<p>Yea all of these colleges should have an area of their site where you can apply electronically. And if you have any questions, just call their admissions office.</p>
