I’m not sure what you mean by “guaranteed med”, but it’s best to apply to Rutgers by Nov 1. In the past they’ve announced admission decisions in waves starting in December (with a small group notified in late November). They will not review applicants until after the Nov 1 deadline.
Not personally familiar with Penn State’s process but I’ve heard it’s similar.
